Acne Scar Correction
Acne scars appear as either rolling depressions in the skin, ice
pick type scars or discoloration (see “Laser treatment for
blood vessels” for removal of reddish coloration). Many people
are aware of dermabrasion as a method of acne scar removal but this
technique which involves sanding the skin down, is suited for only
certain types of skin and superficial scarring.
“Rolling” acne scars are smooth undulating
depressions of the skin and are a result of scar tissue that anchors
or tethers the overlying epidermis. Subcision is
directed towards releasing these anchoring fibers. A needle with
a harpoon like knife edge at its tip is inserted into skin and with
short sweeping motions transects the scar tissue. The area undergoes
remodeling adding bulk and elevating the epidermis. The patient
is asked to stop blood thinners a week prior to the procedure as
this will help reduce bruising. The procedure is performed under
local anesthesia and is very well tolerated.
Ice pick scars are sharp and angulated and are
best treated by punch excision. The scar is excised by a tiny cookie
cutter type instrument or by a scalpel and the “level” skin
brought together by a suture. This process involves replacing one
scar with another but is very effective and can be combined with
either laser or dermabrasion. |
