How to Submit Your Photos
1. Click the Upload Link Based on your Status of New or Existing patient
This will then take you to our secure online form.
2. Complete All Required Information
Fill out the form and any necessary information needed so our team has everything we need.
3. Upload Clear Photos of Your Skin Concern
Be sure to read details on the document to provide our dermatologist a good photo for review.
4. Submit the Form
Once submitted, our dermatology team will review your photos and contact you within 48 business hours with a personalized care plan and next steps.
